In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients may request to see their medical records. Such requests should be made through the practice manager and may be subject to an administration charge. No information will be released without the patient consent unless we are legally obliged to do so.

Helps smokers aged 16 or over, living in Nottingham City to quit smoking for good. The service provides 12 weeks of stop smoking support.

The initial appointment is with a specially trained pharmacist to develop a plan to suit the individual and their smoking habits, followed by follow up appointments with our Stop Smoking Advisors. The plans include a range of therapies including behavioural support and nicotine replacement therapies. Consultations are offered both digitally and face-to-face based on each citizens preference.

Criteria:

Anyone aged 16 or over, living in Nottingham City.

Run by one of our practice nurses who will give advice concerning all aspects of asthma and COPD, arrange breathing tests (spirometry) to assess disease severity and help you improve your inhaler technique.

These are held regularly by Dr. Harte, the clinical pharmacist the practice nurses and the Healthcare Assistants. We monitor the treatment and give advice on lifestyle, diet and how to stay healthy.

Parents are reminded by letter when their child/children are due/overdue for their immunisations. (The red parent-held record book has a list of when the immunisations are due. Please bring it with you to your appointment). All immunisation appointments must be pre-booked with the Practice Nurse.

This is provided by the Community Midwives based at Broxtowe Children’s Centre. The mid-wives provide ante-natal and post-natal services in the local area. They can be contacted on 0115 8833368.
